Media Release, Event Waiver of Liability & Consent for Participation Agreement

This document confirms the agreement between you and Universal Soul Service Corp (a company registered in Ontario, Canada) ("USS").

USS organises events at its locations, external venue and online to bring the teachings of Master Zhi Gang Sha and the Master Teachers (together with its training instructors and guides, the "Authorised Parties") to you and other event participants. By signing this agreement, you acknowledge that you are attending the specified USS in-person and/or webcast event(s) (the "Event") of your own volition; and that as an Event attendee, you voluntarily choose to participate in and partake of Event activities, teachings and offerings, including receiving spiritual healing and blessing services offered by USS and the Authorised Parties as part of the Event and hereby affirm your will and consent to receive the spiritual blessings offered in this service.

Specifically, you acknowledge the following:

USS and the Authorised Parties could offer spiritual blessings, which may assist in the removal of energetic and/or spiritual blockages in the physical, emotional, mind and/or soul bodies of the recipient. They can be offered in person or remotely, over the telephone, without touching, in an audience setting or through products or other services, all of which may be offered at an Event.
You are solely responsible for and prepared to handle any pre-existing medical conditions. Furthermore, if you should require or desire emergency or professional medical attention, you are responsible for all costs associated with your care.
Blessings and practices offered by USS are not a substitute for conventional medical diagnosis or treatment for any medical or psychological condition. For such issues, you will seek the help of a licensed physician or other healthcare professional.
You hereby acknowledge that your participation in the Events and any of the events or programs associated with the Events is of YOUR OWN FREE WILL AND COMPLETELY VOLUNTARY, regardless of anything stated or implied.
You are fully responsible for all decisions you make in conjunction with the Events and you alone will determine which activities you will participate in. You further understand that YOU DO NOT HAVE TO PARTICIPATE IN ANY ACTIVITY AND YOU MAY WITHDRAW YOURSELF FROM AN EVENT AT ANY TIME. You agree that you will use your own free will and discretion, considering your physical and mental health, when deciding if it is appropriate for you to participate in any activity.
You understand that USS and the staff at the Event are not physicians and are in no way qualified to determine what is appropriate for you to undertake in terms of your own personal health situation. You agree that you shall consult or already have consulted a physician relating to your health and that there are no reasons why you should not participate in Event activities.
You understand that there is no promise, guarantee or other warranty of any results from receiving any blessing service.
You acknowledge that all physical and electronic materials related to the services, products, teachings and blessings offered by and through USS, the Authorised Parties and other organizations and affiliates of the USS and the Authorised Parties are proprietary, reserved and/or copyrighted intellectual property. These materials include, but are not limited to books, CDs, DVDs, digital audio and video media and content, text in publications, other than books, text on websites, photographs, other images and other design elements. You therefore acknowledge that they cannot be made, reproduced or disseminated for any commercial or other purposes outside of the work of USS and other organizations and affiliates of USS and the Authorised Parties without express written consent and authorization, whether such consent and authorization are freely given or are through a licensing agreement.
You further agree not to teach others any of the activities conducted at the Events unless you are fully trained and qualified to do so.
You acknowledge that: (i) you have read this document; (ii) you will inspect the facilities, grounds and equipment prior to being involved in any activity; (iii) you will not participate until you have conducted such inspection; and (iv) your participation is your acceptance of the facility, grounds and equipment as being safe and reasonably suited for the purposes intended.
You have voluntarily signed this agreement.

Media release agreement:

You acknowledge that as part of your participation in the Event(s), your voice, statements (including written statements and comments), words, image or likeness may be photographed, filmed, recorded, videotaped, audio taped, transcribed, printed, quoted or otherwise reproduced (collectively, the "Materials").

You hereby irrevocably grant to USS and the Authorised Parties for any purpose, including advertising and promotional purposes, for all media and formats throughout the world (including print, internet, film, radio and television broadcast and/or other electronic medium present) the full, perpetual and exclusive right, to use, publish, distribute and/or incorporate (alone or together with other materials), in whole or in modified or edited part, the Materials, which may include photographs, video footage, webinar recordings or audio recordings taken of you, your voice, statements and/or words provided by you as a result of your participation in or attendance at an event with USS and the Authorised Parties.

You hereby agree that you will not bring or consent to others to bring any claim, action or proceedings against USS and the Authorised Parties on the grounds that anything contained in the Materials or in the advertising, publishing and publicity used in connection of the Materials, is defamatory, reflects adversely on you, violates any other right whatsoever, including without limitation, rights of privacy and publicity. You hereby waive and release any and all claims, demands, actions, causes of actions, suits, costs, expenses, liabilities, and damages whatsoever that you may now or in the future have against USS and the Authorised Parties.

This agreement shall not obligate USS and the Authorised Parties to use the Materials or to use any of the rights granted hereunder, or to prepare, produce, exhibit, distribute or exploit the Materials. USS and the Authorised Parties shall have the right to assign their rights hereunder, without your consent, in whole or in part, to any person, corporation or other entity.

Acknowledgement:

IN CONSIDERATION OF AND AS PART PAYMENT FOR THE RIGHT TO ATTEND AND PARTICIPATE IN THE EVENT, YOU HEREBY AGREE TO WAIVE AND RELEASE ANY AND ALL CLAIMS FOR DAMAGES, INJURY OR LOSS OR OTHER LIABILITY RELATED TO YOUR ATTENDANCE AND PARTICIPATION IN THE EVENT TO THE MAXIMUM EXTENT PERMITTED BY LAW AGAINST USS AND THE AUTHORISED PARTIES EXCEPT IN RELATION TO DEATH AND PERSONAL INJURY RESULTING FROM AN ACT OR OMISSION BY USS OR THE AUTHORISED PARTIES OR ANY CLAIM CAUSED BY: (i) THE NEGLIGENCE OF USS OR THE AUTHORISED PARTIES; OR (ii) A FAILURE BY USS OR THE AUTHORISED PARTIES TO PERFORM THE SERVICES WITH REASONABLE CARE AND SKILL.

The foregoing applies to the Event for which it is signed. Your participation in the Event and the services offered was not solicited by USS or any of the Authorised Parties.

You agree that this Agreement and any dispute or claim arising out of or in connection with it, its subject matter or formation shall be governed by the laws of England and Wales. You irrevocably submit to the exclusive jurisdiction of the English courts to settle any dispute or claim (including non-contractual disputes or claims) arising out of or in connection with it, its subject matter or formation.

You have read this Agreement and acknowledge receipt of the individual documents listed above. You understand these documents and agree to their contents and to be bound by them.
